I need an application form mod created for my vbulletin website and i will pay up to $200

although there is an application form which i am using, its very outdated and not customizable and the layout looks very untidy.

Here is the following information:

The application form should be based on world of warcraft and be highly customizable. i want to be able to add and change questions as and when i need to, the game is progressing and so the needs of more questions would become relevent...

the idea is that a visitor fills out the application; clicks submit and it gets posted in a forum [specified in the control panel] in its own unique thread which can be replied to by guild officers.

i have created 2 text document files with the all the information i need added to this mod which i can send to you.

The cheaper way would be to just set up a forum where members can only see their own threads, and own replies. Many sites do this so people can talk privately to the staff or whatever usergroup is assigned to that forum.
